PS4: Game uninstalled on its own

Loaded up BFV yesterday and on the startup it crashed.  Went to restart and found the game is no longer in my installed games.  I can find it in the "Not installed" list with a download button being ...

    Have you tried completely resetting the PS and deleting all data?
