Summarize your bug I died. Asked for a revive. Could not choose option to die or ask for help menu didnt pop up. Was in the middle of the enemy so revive wasnt quit possible.
How often does the bug occur? Rarely (0% - 9%)
Steps: How can we find the bug ourselves? I think you just have to die allot the bug is really rare. The game mode i played was Grand Operations but thats not an option on this website.
What happens when the bug occurs? I cant choose to die so i cant respawn. Thats what made me leave the game cause nobody could revive me.
What should be happening instead? An option in the menu to suicide.
Well its pretty simple just add an suicide button in the in-game menu and its fixed.

The team made this fix in the update that's coming tomorrow.
- Fixed a bug where players were unable to redeploy if they had been killed by Dynamite and
then almost got revived by a medic

Just happened to me on PC while playing Breakthrough on Twisted Steel.
Got shot, one medic tried reviving me and got shot in the process, I tried to right-click out, second medic rushed in and started reviving only to get shot immediately, was stuck in the downed state and couldn't Redeploy or do anything else.
