Controlled character cannot use health potion

This seems to be a bug or something, but I can’t wrap my head around it. It’s been happening with Dorian this entire session, and it also happened with Blackwall and Bull earlier.

On the radial menu, the healing potion is greyed out and the left and right slots (Regen and Healing Mist for Dorian) are available. There’s only one healing potion missing and my use-til limit is set to 2. Whether the affected characters are healing when they aren’t controlled is up in the air—with this headache, I could be missing it—but when I’m controlling them, they can’t access the healing potion. Has anyone seen this before? If so, how can I fix it? I reloaded about a dozen times now, traveled between camps and maps, switched party members, etc, and this keeps happening.
