I've had this exact same thing happen to me! I've completed the game twice no on Immortal Difficulty and still hasn't worked.
I tried uploading save file to cloud storage, deleting it off console. Reinstalling the game and downloading the save file (this has worked for other games) and it still hasn't worked.
I've completed the game 3 times now in 6 days. I'm done

Not sure if it has anything to do with an issue with settings menu as every time I press start and go into settings, it's asking me if I want to revert my settings?

@NonprofitGenie If it keeps happening, reset to default settings and reboot the game. You can also do the following:
- Manage your downloadable content (DLC) on PlayStation
- Clear the systems cache (Close the game completely > Turn off the console > Wait 30-60 seconds > Restart and Relaunch)
- Verify you signed into your PSN account
- Check for updates
- Restore Licence
