I am not getting any house rules headliners rewards after level 1

Summarize your bug Didn't receive Level 4 award (84 Ovr BND Headliners Hero Fantasy Pack) from House Rules after reaching the level.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Unsure. I had a hard crash of the game after one House Rules game (I think I lost that one), and later someone disconnected and it took awhile to register the victory. That might have screwed up the accounting of the points towards the level progressions. I'm also terrible at this game, so I had many, many losses which garnered only a few progression points.
What happens when the bug occurs? The Headliner pack isn't awarded. I received one 84 ovr headliner pack from the Level 4 Headliners challenge. I assume multiple packs were possible.
What do you expect to see? Receiving the pack as per usual.

Edit: I see someone just posted a generic version of this issue as I was composing this.  I'll leave this report in case it provides additional info.

I haven't received my house rule items since level 3. Also i did not get my pre-order content. I've tried to go with it hoping i receive my items the next day but nope . Ea or anybody please reply. Im becoming annoyed! 

Finished every level from house rules, didn’t receive any reward from it. Please look at this, want my 40k, 84 bnd and 3x80+ packs

I didnt get my pre-order stuff and also didnt get my house rule rewards should have 3 80+ and my 84 but i dont 

Played enough again this morning to pop to the next level after the update.  Level 4 didn't pop.  Not sure if I got the level 5 award of 10k coins either.  Sigh.

I am level 6 in house rules have not no rewards
