Ball Carrier Throws Ball BEHIND him!!!

Hi, I'm on PS3, playing Madden 16.

My ball carriers keep randomly throwing the ball backwards!!!  It's so frustrating!  It's especially awful on punt returns, but happens almost just as much for running backs and wide receivers.  Also happens now and then on defense after I've made an interception.. I start running the ball back and fling it behind me.  At first I thought it's because I pressed something, but in testing that theory, I pressed all the same buttons in the same scenario later in the game and nothing happens then.  At various times I've thought it was R2, R1, or some direction I press in the left or right stick.  I've resorted to returning punts directly up the middle for fear of activating whatever error is happening, terrifying my opponents with my repetitive 12-15 yard returns.  It's really terrible that every time I play I can count on one of my players wildly hurling the football 10 yards behind me for a turnover.

PLEASE help.  I even tried to ask Madden Support "experts" and they said the only place I could get help was here in the forum.
