As I've also worked for other gaming companies, live production servers can produce strange outcomes. When testing on internal servers, you don't have hundreds of thousands of players available logging in online, so that could be one factor on why some things don't get picked up through testing. Bugs are common in software and we apologize if the amount of unexpected issues at launch is making your experience less enjoyable for your expectations. Our role on these forums is to gather all the bug reports summitted by the community and alert the Madden team what is happening. This includes high priority issues such as the "server cannot process your request error" which we have another main topic on where that can be discussed. Sadly, it's a more difficult issue to address than anticipated. They are still investigating it and we'll share updates as soon as we can. Again, I apologize for the frustration you've experienced and thank everyone here helping the team address as many bugs as quickly as possible.

This was an issue identified at the launch of UFC 4.

I think @IccyLlama is onto something with it being related to the cloud. Would you be able to change your profile picture to a standard Microsoft default profile picture? It also could be worth deleting your Gamertag from your system and then re-adding it with a default picture to clear up any corrupted Gamertag cloud data.
If you can get back to me with this info ASAP that will be helpful in ruling out that possibility.
