Summarize your bug My defenders randomly blitz in zone or man coverage leaving their man open. This happens a lot with linebackers making it impossible to cover TE's and occasionally with cornerbacks and safeties. This occurs in multiple cloud franchises and offline franchises.
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? I see it occur in 4-3 and 3-4 defenses. It also happens in Quarter defense. In Nickel and Dime, the corners or safeties will blitz. This happens in man or zone coverage.
What happens when the bug occurs? It leaves the receiver wide open for easy completions leaving gaps in the defense.
What do you expect to see? I expect to see the defender guard their man in man coverage or play their zone in zone coverage.

I play the PS4 version on the PS5. However, the same bug happens on the Next Gen version too. This bug was also an issue when I played Madden 21 on my PS4 and it carried over to both version on the PS5.
