Gameplay bug(s), ran in for a touchdown, downed at the one and can't challenge

Summarize your bug I have had multiple issues of gameplay. I just ran in for a touchdown with the ball downed at the one, clearly on his feet in 3 yards deep before falling to the ground and still downed on the 1 with out the ability to challenge the play. I have had forward passes been called as fumble with no ability to challenge. The biggest issue with that I have had a forward pass get knocked back 20+ yards called as a fumble with defenses always picking it up as my offense doesn't respond. I have had caught in bounds been called out of bounds with no ability to challenge. I have had forward progress been called down 1-3 yards off with no ability to challenge. I have had WRs that are often times wide open not even try to go for the ball when either the possession or the aggressive catch options are pressed. I have had many, with the ball only a yard or two in front of WRs, go by with the WR only going for the ball after it has hit the ground. I have also had, in the few instances I can challenge, it be called incorrectly. Ball was clearly out before player hit the ground but was reversed as down and they got it back off of a punt fumble I recovered. Same was true when player was clearly down but still called a fumble. Most of these plays are unchallengeable but when I can it is called wrong.
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? Look for videos online, I will also start recording most of my gameplay as well. Also simply play the game.
What happens when the bug occurs? Challenge play is disabled with the, nothing to challenge message.
What do you expect to see? The issues fixed. Ability to challenge accurately. I remember older versions of Madden NFL and even NCAA Football very accurately spot the ball and call challenge plays accurate.
