I'm having the same problem with a brand-new Xbox X. I have the Madden 21 disc version so I installed it and updated it. Tried to open it and title screen shows for 2-3 seconds, then screen goes black and nothing ever happens. I have EA Play so I tried getting the free trial to see if that would work but the same thing happens. I uninstalled the disc and re-installed but same thing happens. Over and over I can only get as far as the title screen and then a black/frozen screen. I do have NBA2K21 and that works with no problems at all.
I even downloaded Madden 20, Madden 19, and Madden 25 from EA Play, but none of those work either. Those older versions let me get past the title screen and pick teams or settings, but it freezes before the actual football game starts. Once yesterday Madden 19 actually started, but it was some kind of demo and there was no sound. Nothing but problems no matter what I try.
Same! We have NBA2k1 and it works fine. So does FIFA21.
Is there anything unusual about your setup?

Nothing unusual, actually I have tried multiple set-ups. Tried 3 different TV's (basement, office, family room), tried different HDMI cables, tried both wireless connection and hard-wire to modem. Even returned the disc to Target and got another one. Cleared the Xbox cache, tried to play offline, set-up different Microsoft accounts, nothing nothing nothing.
Yesterday i downloaded Farming Simulator and some Fishing game and neither one of those will work either. So it seems that anything I download off Game Pass will not work (I have Game Pass Ultimate). I am slightly worried that the Madden 21 Trial is blocking my actual Madden 21 I have downloaded, but I tried the Madden 21 disc two days ago before I ever downloaded the free trial and this same problem was happening.
NBA2K21 running like a champ but would like to play more than 1 game! I'm tempted to go buy a different game from the store like Cyberpunk or something and see if that works.

I found a few other people on Twitter with the same problem.
I wish there was a way to see crash logs on the Xbox but those don’t seem to be exposed to users.
Not sure where to go next to find a resolution.

Turns out that you need to get a digital code in exchange for the disc. Here are the instructions you should follow:

I am in the process of getting the code now after having gone through the gyrations required.
