Madden 15 access code

We bought the playstatiion 4 for a christmas present and purchased the madden 15 seperately. When we we opened the game there was not a manual or access code for the ability to play online with the madden ultimate team. How can i obtain a code for the game that was purchased at full price of $59.99 from target ?
