Can you reply to us with a DxDiag so we can review your system specifications and let us know the exact Xbox controller model you're using?
Make sure you check to see if you have the latest controller driver update on your PC. If a controller doesn't have the latest driver update it can create hardware issues like what you're describing.
Have you checked accessibility and game settings in game for vibration?

My controller was working flawlessly with FIFA20 and Madden20. With vibration. Xbox One Controller elite series v2 it is. It is working with other games such as 'Wreckfest' or 'For Honor' as well.
In Madden21 it doesn't work, no matter if I set vibration to 'normal' or to 'assist' in ingame menu.

Standard Xbox One controller tried both wired and wireless.

Steps: How can we find the bug ourselves? The vibration on controller doesn't work in any game mode on FIFA 21. Whenever you hit post, bar, hard tackle, the auction bid is ending in FUT, when you turn on Timed Finishing the controller should vibrate when you green time it, etc. None of the above is happening, so it's easy to verify. Tried different controllers, still no dice.
What happens when the bug occurs? The controller does not vibrate at all.
What should be happening instead? The controller should vibrate on hard tackle, hitting post/bar, when you green time Timed FInish, the acution bid is closing to an end.
As stated. There is 0 controller vibration in FIFA 21/Madden 21. Please be aware of this issue. It is there since the game launch!
