Plenty of issues in Madden 15 need fixed...here is a whole bunch of them! ~
First off, I'm gonne leave this here for all the EA people to see how much your game needs to be fixed. Take a look at them and realize people who play offline need just as much attention as those who play online or MUT (please actually do something about them...and no, NOT by doing it in Madden 16):
Now, onto my own personal bugs (all of these in the PS4 version):

Bug 1
Whats the bug? Instant Replay in is always locked
What mode did you experience the glitch/bug in? Offline CFM (as a player)
How did it happen or what did you do to trigger it? Can you recreate it? When you are off the field, there is no way to see the replay as the "Options" button does not respond. It can be repeated.
How often has the bug been reproduced? Everytime I'm taken off the field and I want to see a replay of the last play
Bug 2
Whats the bug? End of game highlights are glitched/missing players
What mode did you experience the glitch/bug in? Offline CFM (as a player)
How did it happen or what did you do to trigger it? Can you recreate it? At the end of the game, when looking at highlights, some of the highlights are missing players (and no I'm not talking about plays with no highlights). I have one where Joe Flacco is invisble and half the defense is gone.
How often has the bug been reproduced? Twice so far.
Bug 3
Whats the bug? Being made to watch a play I am not a part of
What mode did you experience the glitch/bug in? Offline CFM (as a player [RB and DT])
How did it happen or what did you do to trigger it? Can you recreate it? Randomly during the game, I will click "Sim to next player play", it will sim until it goes into game. It will chose a play and I will not be on the field and forced to watch the play unless I waste a time out.
How often has the bug been reproduced? Usually about 10 times a game
Bug 4
Whats the bug? When playing as a play that is not the QB, the ball is never snapped and results in a Delay of Game
What mode did you experience the glitch/bug in? Offline CFM (as RB)
How did it happen or what did you do to trigger it? Can you recreate it? A couple of times a game, I will line up for a play and the ball will never be snapped. Once, Flacco stood under center for 25 seconds on a HB Slam (read: no other motion before the snap) and the play clock ran down to :00 resulting in a Delay of Game. This happens once or twice a game and forces me to waste a timeout.
How often has the bug been reproduced? Once or twice a game this happens.
Bug 5
Whats the bug? Stadiums only sell exactly 50% of seating in playoff games
What mode did you experience the glitch/bug in? Offline CFM (as owner)
How did it happen or what did you do to trigger it? Can you recreate it? When my team hosts a playoff game, I will only sell 50% of my tickets. Exactly 50%. Nothing changes it. And it happens at every home game.
How often has the bug been reproduced? Every time my team hosts a playoff game
Bug 6 & 7
Whats the bug? (Bug 6)Replays and TV graphics cannot be skipped in between plays, often resulting in pushing the button that brings up a "spike". (Bug 7)Cannot audible out of spiking the ball.
What mode did you experience the glitch/bug in? Offline CFM (as coach for both)
How did it happen or what did you do to trigger it? Can you recreate it? (Bug 6) In between plays, replay and TV graphics will play that cannot be skipped unless you push the "O" button. Pushing the "O" button results in no-huddling a spike the ball play. (Bug 7) For some reason, I cannot audible out of a spike play and thus have to waste a timeout.
How often has the bug been reproduced? Almost every time a play is over and there is no no-huddle called.
Bug 8
Whats the bug? Relocation team names will change between weeks
What mode did you experience the glitch/bug in? Offline CFM (as owner)
How did it happen or what did you do to trigger it? Can you recreate it? When I am prompted to choose a name for a team, I will choose a name. When it is time to choose my uniform, the team name has changed. For instance, once I chose the "Austin Bats" and when it was time to choose the uniforms the name had changed to the "Austin Armadillos"
How often has the bug been reproduced? It's happened once, so far.

There are also a ton of issues with the way the normal gameplay is: Why is there no option to just skip the TV graphics at half time? What is up with owner mode forcing bonus money to be paid out of funds? Why the heck is there only 17 options for cities to relocate to (whatever happened to giving us the freedom to relocate a team to Honolulu or Fargo...maybe some of us have some creativity)? Along with that, why does each city only have three team names and logos (let us be creative and stop making us feel like kids by telling us what we can and can't do)?
As a huge football fan, I feel like I am ignored because I don't play "online" or MUT. I understand that is a trend, but there are a large portion of us who want to play this game offline. I hope you see these issues and realize you actually need to focus on all of your players and not just the "popular" ones. I hope something is done soon to fix this game or you'll lose another customer.
