PS4 Disk Error - Cannot continue using the application

I got Madden 15 for PS4.  It gets to the "It's In The Game" intro then crashes.  "Cannot continue using this application".  "Data is corrupted".   I have deleted and reinstalled 20 times.  I returned the disk to Best Buy and exchanged for another.  Same issue.  I reset my PS4 completely.  Same issue.  I reinstalled latest PS4 update.  Same issue.   The game just keeps telling me to delete the application and re-insert disk. But the same problem keeps happening.

I have contacted Sony 3 times and they told me to contact EA.  

Anybody have this issue?  So frustrating.
