@bkvb2012 I've seen this bug too a while ago but cannot remember the play or formation.
my first guess was that this might happen if the assigned WR for a position in this formation is somehow mixed up between WR1/WR2/WR3 etc. and SLWR1/SLWR2. in other words, if one player is assigned both to e.g. WR1 and SLWR1. maybe replacing the SLWR1 with another player might solve the issue for a specific formation? but I have to say, that is only a shot into the dark...
I'd guess it would be very helpful for the dev team, if you could take some notes if this bug occurs (if feasible) and provide it here. I think it might be good to know: 1) which offensive formation, 2) which play, 3) which players are assigned to WR1-WR5, TE1-TE3, HB1-HB3 and SLWR1-2. the last point might be easily answered by a screenshot of your lineup.
