I really think the Bottom line of the issue is the same as : https://answers.ea.com/t5/Bug-Reports/Field-Goal-Block-Penalty-Glitch/m-p/6965387#M70 .
The game Don't manage properly penalties or reviews after the turnovers and the states it come back to when it happends is wrong. I would guess that not everything is reinitiallized in the state (like the number of downs, the distance for instance)
