I've managed to get in touch with someone from Level 3, and they've said they'll look into it for us. I'll post when they get back to me with more information/a resolution.

Had a few games last night at 1am and it's still working at 8am this morning. UOTrace results have also been consistently good.
Very promosing and I'll try again late this afternoon to be sure.
My traceroute has improved, but it still takes about 5 minutes on startup when "Connecting to EA servers", the Multiplayer menu takes 2 minutes to load and the store is empty, and I get regular disconnects. I'm running it on a beefy system, installed to an SSD, with no firewall and my PC in the modem's DMZ. Something is still wrong.
It's a start, at least Level3 is aware now. Nice work :D
