Looks like you're playing through Origin, maybe it's just a console issue.

It's not just a console issue. I can't get it on pc, it has to be linked to the app, which I only used like once and haven't used since then. Leveled at least 5 teams to 20 in game, no achievement.
I did manage to get the one for having 5 teams though.
