Goalie Pulled at Start of Game Then Crashes!

I jump in to squad battles today and I have 6 guys on the ice at the start of the game for the faceoff and the goalie is PULLED!

Never saw this one before!

As soon as the puck drops the game freezes  and crashes to the Home Screen.  No matter what mode I play (squad battles, moments, wild card, etc.) the game freezes crashes at the face off.

I have tried all the fixes on the internet for Xbox:

1. Power Cycle Restart
2. Cleared Alternate MAC address
3. Cleared Local Storage
4. Cleared NHL 25 Cache Files
5. Cleared Persistent Storage
6. Removed and Re-added Account    

Nothing worked. Is anyone else having this issue? EA how do I fix this?
