Controller disconnects mid game and when trying to reconnect cant do anything

How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Xbox elite controller: Click on LB and raise the right stick simultaneously. Happens 50%. Click on RB, LB and raise the right stick simultaneously. Happens 90%. Click on RB, LB and raise the left right stick simultaneously. Happens 100%.
What happens when the bug occurs? Controller disconnects mid game and when trying to reconnect cant do anything on NHL. Can't move my players. But I can go in the Xbox menu.
What do you expect to see? Correction please. It kicks us out and we can't do anything in NHL when trying to reconnect.
