Do you have any corrupted save files lingering for the game on your system?
Whenever we've came across these before it's came down to the controller, which is why I recommended trying out a brand new third one to see if there is any impact (unless the 2nd one you have was also brand new).
We could always go the route of factory resetting the console, but I really don't want you to go to that extreme unless you don't mind. - @EA_Blueberry I have no idea about the save corrupted files
