  2. Account Name: foster711
  3. Timestamp (date/time, with time zone): Friday sept 13 8:15pm EST
  4. Which mode were you in?: ones, but has happened in threes and online versus as well
  5. What were you doing just before it occurred?: turned on my Xbox 
  6. How often does this issue occur? Is it 100%, every other game, etc?: every time I play, factory reset, both yesterday and today
