  2. Account Name: CuvvahThaSpread (gamertag), account name is drewredux for EA/Origin 
  3. Timestamp (date/time, with time zone): all times, EST
  4. Which mode were you in?: HUT, trying to go into an offline HUT challenge
  5. What were you doing just before it occurred?: trying to do the above
  6. How often does this issue occur? Is it 100%, every other game, etc?: all the time
  7. Can you attach a picture of the error code or provide the error code number? Please see attached
