Has anything been done to fix this problem?  

This is happening in CHEL mode for me.  I reset my XBox and nothing seems to work.

    IF you have a extra controller or a usbstick for GH or something else plug them out. Also check that you have open NAT in network settings.

    if you have open NAT, and you have plug out everything. Restart Xbox with only network cabel and AC cabel plugged in.

    Start with a wireless controller only one signed in.  Or have just one controller plugged in.

    you can control this in offline mode in switching sides in a game

    Work fine online after this
