3. Since the week 14 World of Chel challenges were released and everyday since.

4. World of Chel

5.Loading into World of Chel attempting to claim rewards from the weekly challenge card

6. Occurs 100% of the time

I have tried every "fix" and "solution" recommended on these forums. Have gotten to the point that I just assumed it was a glitch and was going to need to be patched, I have missed out on weeks of potential gear at this point and there is no possible solution on my end.
