Taking a quick skim of your account, it looks like it has a copy of Titanfall, so the code went in. It won't go in a 2nd time, giving a "the code has been used" error. Check that your My Games tab in the Origin client is set to View All, or even View Hidden. If it's not there, click Origin in the top left, then Reload My Games.

If it still doesn't show up, exit Origin from the menu, and restart it."

    Hi there, I've taken a look at your account, beznit, and I can't find a copy of the game. If you used to code on a different account, check the steps above. In this case you'll want to contact your retailer, there may have been an error in code distribution.

    Also check for standard code confusion issues such as mixups between 1/I/l  or O/0 and so on.
