SRS Fire Banner not unlocked

Beat the entire campaign on Master difficulty, earned the achievement on steam, but I have not received any banner other than the base SRS banner. I have double checked again, and even completed the final mission twice, on master. Still no fire srs banner.

    Forgot to add second image, but I have it completed on Master, and never received the Fire SRS banner for completing the campaign
