Happened to me a few times, but only after I binged played for hours...I think in may case the console was overheating. Much more often I get "server error - client desychronization".
I come to think, that Titanfall is just not suitable for the 360 and all the three amigos (EA, MS and Respawn) have created is an angry customer (at least in my case). Come September, I will upgrade to a PS4 and Destiny.
