It was a steering colum failure on his car. He didn't like the hight of the steering wheel so instead of removing the whole system and replacing to his liking which they would usually do, but to save time some idiot basically just cut the steering arm,minimised it and then rewelded the piece back together. Totally not structurally sound. If you watch the "senna" movie,just was release I think two years ago you can see the actual modification done to the steering colum, you can see the welded joint,still going through court ATM, and the Williams f1 car he was killed in still in possession of the authorities as to why it wasn't simply replace properly. Trully disgusting, when you watch him on the grid right at the start of that fatal race, he knew something wasn't right about that car, he had this confused look on his face, I've rarly seen him do, but he needed the points as he hadnt scored points in the first 3 races as his rival Michael Schumacher had won all three. So he was desperate to score maximum points.
