I also experienced the same thing multiple times. I was using the M2010 ESR with the Compensated Break, 26" Carbon Barrel, 5RND magazine, Match Grade ammo, DLC Bolt, Anti-Glare Coating, R-VPS 10.00X scope, playing on PC through Steam.
