If I throw a grenade but don't watch it...

Sometimes if I throw a grenade but don't watch it, not sure it explode? A few times I've fire RPG\ tossed a grenade and because I'm jumping behind cover as I do it I don't hear the explosion\ no sign anything was blown up.

I have ask if we have the old disappearing ordnance bug?
