Not being pulled in to matches on Xbox
About 75% of the time when I am partied with players in a squad after a match I am not brought in to the next game.

Still? Dang - this was a problem with me (PC) and my friend (Xbox) when the game first came out. I'd queue up and he wouldn't get pulled in about 50% of the time.
Something wrong with matchmaking. Nine times out of ten, a new game doesn't spin up and I have to leave matchmaking and go to the main screen to find a new game. What is the problem with putting all the players into a new game.
