Revive glitch

On multiple occasions I’ve ran into the following glitches concerning revives. 
-If either you or your teammate gets downed in destroyed building location you get “stuck” in the building wall and you cannot revive or get revived, there is no “hold X to revive” que.

-I was downed and when my body hit the floor I fell into the under side of the map, which resulted in not being able to be revived at all and it counts as an automatic death.
