This has actually been an issue since as early as last year, but as it is right now, if you have more than three players on defense with X-Factor abilities, it will cause 1-2 of those X-Factors to glitch out and not be activated, nor capable of being activated or swapped. I discovered that after reducing the amount of X-Factor Players on defense to three or less players, this no longer becomes an issue, and you are then capable of switching , swapping, and activating X-Factor abilities as normal. The frequency at which this occurs seems to be pretty much anytime you have more than three X-Factor players on defense. This does not occur on offense that I have found.
