I have set Controller Volume to 0, and sound mix to Headphones, the slider keeps resetting Controller volume in every game. When the sliders do save for a game ir two, sound mix is missing skate and stick sounds that come out of controller.
Headphone mix should always mute controller…
Ideas
Controller Volume won’t save to 0

Whether I set it in the Hub or in-game, the next game I join it's back at 10. I set mine t o1 because it's just obnoxious at full volume. Why would I want to hear the music of my opponents goal horn at full blast?

I have also run into this bug. Please fix this. It's so annoying!

If this is in CHEL, I can concur with this.
I've only just started working in my CHEL character (because HUT objectives and rewards aren't offering much since I finished Wildcard this week), and each game I get blasted with by my controllers volume.
Luckily I'm just playing ProAM, so I can pause it to reset it to a reasonable level, but if I was playing real games that would be a complete distraction or downright unacceptable (like if I had a family and needed to be quiete at night, or something like that).
And I've also tried setting this using the in game and main CHEL home "Play" menu's settings, and the controller volume seems to be the only one that doesn't save. I turned off auto back-skate and that saved fine.
