Defensive Assisst grappling is not working

The Defensive Grappling Assist is not working correctly. Even with the option enabled in the settings, the defensive grappling prompts and assistance stop working during fights.

This happens even when I am not playing online and not playing on Legendary difficulty. I have tested it in offline modes, such as Quick Fight, and the problem still occurs.
