I never saw the hot tube death yet, but did see a sim die in the swimming pool, grimmy walked over to the edge of pool, wave his hands then poof.. urn. then right after had a 20 sim mins swim. then poof again... gone.. it could have been a one time bug.
@alanmichael1 There was a bug with Sims getting stuck in Hot tubs while woohooing, it's possible that death are still bugged.
I currently don't have much time for testing myself but are you able to reproduce the same behavior a second time ?
I tested this on my own lot although home lot and it all went fine. Sim went out of the tub and died. Reaper came.
Was it a one time thing for you or are you able to reproduce it a second time ?

Can't reproduce either, tried 3 times. Once she died, got out as a ghost Grim showed.
2 other times he died after getting out and Grim showed.
