Adopting Child messes up relationships

Steps: How can we find the bug ourselves? have your sim adopt a baby/toddler/etc. and have your significant other adopt that child.
What happens when the bug occurs? I had my sim (Ava) adopt a toddler (Bria). Ava had her girlfriend (Hilary) move in. I had Hilary adopt Bria and a bunch of stuff got messed up. Ava turned into Hilary’s sister and Bria was now was her niece.
What do you expect to see? I expect the relationships to stay the same, and the sim who’s adopting that child to become the other parent in the family tree.
Have you installed any customization with the game, e.g. Custom Content or Mods? On console - never used.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
When I went into cas.fulleditmode it wouldn’t let me change any relationships either, so I had to quit without saving
