I'm having a similar problem with a residential rental I placed in Forgotten Hollow. So I want to confirm that the problem isn't necessarily restricted to Willow Creek.
I will try to post an image when I get home later today.
Edited to Add: This residential rental lot is also having the "error code 132" issue I've seen reported elsewhere on this site (where players try to visit a lot or rental unit in live mode, and then are kicked to the world select screen, presumably due to corrupted objects). I plan to try to fix the problem using solutions I've seen shared in this forum.

Hi Crin, my issue is identical to OP's with the exception that it's occurring in Forgotten Hollow instead of Willow Creek. Here are details:
Product: The Sims 4
Platform: PC
Which language are you playing the game in? English
How often does the bug occur? Every time (100%)
What is your current game version number? 1.106.148.1030
What expansions, game packs, and stuff packs do you have installed? all
Steps: How can we find the bug ourselves? Place the "Grand Castle Estate" lot in Fledermaus Bend in Willow Creek. Purchase as Straud, convert to a Residential Rental, decorate the interior, subdivide into three units, move in some households (e.g. the Noble, Rebellious, and Virtuous Vampires). (You can find the lot I created on the gallery as "Vampire Castle Estate" by Cyberpreppy.) Play normally by playing the tenants or having other Sims try to visit the tenants.
What happens when the bug occurs? After the bug occurs, the original building on that lot will still appear when viewed in the world map. Even when demolished first hand. The image of the residential lot seems to be added to the map's background, appearing behind the square that designates the lot location, and behind any other lots placed in that location. Other lots placed in that location can be played normally, even though they appear superimposed over the now-gone residential lot.
What do you expect to see? When the residential lot is removed from the location, it should no longer appear on the map.
Have you installed any customization with the game, e.g. Custom Content or Mods? Yes, many. The bug persists after removing mods.
Did this issue appear after a specific patch or change you made to your system? It appears due to the For Rent expansion.
I've attached a better image of the problem. It's the map of Forgotten Hollow, cropped to focus on Fledermaus Bend. You can see that the building (I've been thinking of it as the "ghost castle") seems part of the background, behind the square lot indicator.

Thanks! I'm still having this issue as well, I do play with mods but as the original post mentioned, I removed everything, though since I used them in that save idk if my save would be useful. I saw that there was a big patch and thought, oh! Maybe it's fixed but alas.. when I selected that save file that I havnt touched for months.. not only was the green exclamation still there over the willow creek lot aforementioned, but it's now also spreading like a virus, orange and green exclamations over other household lots just as many replies have also mentioned.. all I know for sure is that, unlike the original post, this happened as I was filling my world with gallery households so there'd be more sims in town.. it could just be a coincidence, but what likely is not a coincidence is that it happened once I filled the willow creek lot everyone is talking about. unfortunately my game is near unplayable.. everytime I go into build it crashes no matter what lot I'm on. I've tried everything the original post tried to fix it.. but then just gave up and felt that I'll never be able to play again.. my previous save is more than a year old.. so I can't stand the thought of playing it.. and I did start a new save but it hurts so bad to play in a world with NONE of my families or lots I've had since sims 4 first came out.. so I can say I didn't see this problem in the new save but I can't really vouche for that piece of evidence too much.. I've only played maybe 5hrs in the new save before abandoning it out of boredom.. its truly disheartening to see how old this problem is and suddenly I have it and can't play at all.. I've never even sought out help here for having a broken game, I've always been able to fix it or wait for patches.. but this.. just cannot be fixed, so here I am.. I've been playing sims since hot date was around lol but now what.. the only shread of hope I have is that you guys said you put a team together to help.. so please.. please help me play this game I've loved and played actively for more than 19 years now, this is the first time in that 19 years that it's been so absolutely broken that I cannot fix or play it.. at all. 🙏 please help us newly assembled sims bug team 🙏 apologies if I posted this in the wrong place

I am having this problem still, on the latest version. No mods because I play on PS5, but that also means I don’t know how I would share a save with you.
