I also have this problem over the last several months.  Even when I choose the cheat to tan my sim when my sim is nude, the tan lines of the default underwear are obvious.  It seems the Sims government has banned nude sunbathing in a most Orwelian way - they call it nude sunbathing if you are wearing underwear and do not allow fully nude sunbathing.
