Missing dog tag in the Kunlun Mountains mission!

I'm playing on XBOX360 and I can't get one of the dog tags? I've picked up 3 tags but in the startup menu it says I only have 2/3? Can't get the achievment for collecting all dog tags and weapons because of this! Plz fix!

    Dont know if its luck but BF4 has freezed almost every round exept from maybe 4-5 rounds and im now lvl 10. BUT 2 days ago i clocked my RAM from 1600MHZ to 1333MHZ and Disabled Visual theme in BF4.exe properties and have had NO crashes in these 2 days, lost count how many rounds ive played but can guess 12++ in a row without crash...
    I will try some more today. Crossing my fingers :=)
