Old issue, sure, but apparently never properly fixed. The quest description still says to deploy agents to locate Maddox, BUT the marker on the table is labeled "Track Down Samson" and the description of the operation is about locating Samson and is written so it suggests that you've already located Maddox previously. Completing this operation does however fulfil the quest objective.

This is with the Game of the Year Edition from Origin and I haven't touched the main quest after escaping the Fade following Adamant.
