Re: Dragon Age Keep still not updating decisions correctly

Are you looking at your playthrough states (which you can copy to editable states in order to see the tapestry and choices) or states you have created (and thus have no uploaded Inquisition data in them)? 
Also, in the tapestry, if you click on the 'world' icon (top right corner) and go to the settings tab (looks like a gear), you can choose to show 'spoilers' for Inquisition, this should show all available options in the tapestry.
