DAK/DAI can be finicky. I had uninstalled DAI just before installing MELE, so I had to re-install to see if I could get the world state to load. Just finished talking to Varric and it sounds like it worked..Bethany is alive and a Warden. The Codex said that the Circle was annulled, which is not the default choice.

Then making sure you get the "Custom World State" card.

The default World State card looks like this...

(I disconnected from the internet while creating a character to see how the game would react. A notification popped and I had to go back to the Main menu to log back in.)

Thankfully there's a "Back" button, so you can go back and try uploading again.

Note: If the upload is unsuccessful (dates don't match or something else goes wrong) the Back button will become available after clicking "Accept" for the "World State Import" dialogue box.

    Then I will try to Uninstaller it and install it again. But I still don't get it bequause the date matches and I had the dragon card. 

    Or could this be because I play via origin?

    An Additional Note:

    There is an unofficial program/mod for PC that can open DAI save games, DAI Save Game Plot Editor. As with all mods/3rd party programs there's a risk involved and always make a backup before changing anything just in case something goes wrong. That said you can use this program to verify World State choices without having to play through to Haven.

    You do have to play the game for a bit; when I tried opening the chapter save from the very beginning of the game it would not load and the save I made before talking Roderick and Leliana on the bridge wouldn't load. The save just before jumping down to close the breach in the temple grounds worked perfectly. World State choices can be seen in the "Plot Boolean Flags" section and are at the bottom of the list...

    Thankfully the program labels which game the plot flags are for (DAO, DA2, DAI)
