Reported this issue after Title Update 4, along with numerous other users in this community, and still, no response from EA Community Managers.

Ive also had this issue numerous times. It seems to appear when I'm in the lead and controlling the game comfortably then when it appears my player don't react the same and start facing the wrong direction and passes and shots are off direction aswell. Then I end up losing
