Try rebooting your network and power cycle the console. This is a great first step to seeing if things improve. If you're on a WiFi connection, try going wired for a few games or a weekend just to test and see if the root cause could be related to the WiFi bandwidth. I have two channels on my router, so switching to the other one might help. We have some extra steps below in the link via a help article from our site. I've seen connectivity improve after updating the router's firmware to the latest version (you have to login to your router's admin panel).
Reboot the power to the modem/router
- Unplug the power cable from your modem/router for 5-10 minutes to refresh the network.
How to power cycle your PS4 console
- Touch the power button on the front of the PS4 for at least 7 seconds (until the system beeps twice).
- Turn it back on after 1 minute. Launch the game.

Would you be able to run a UO Trace from a PC via this link below so we can look how your connection is reaching our servers? Please feel free to private message me the UO Trace results because your IP Address will be publicly viewable if shared here.
