I had the same issue. It's when you forget to talk to him right away. The NPC disappears after a while and the mission can't be completed.

This bug existed in vanilla, but it would be nice if it could be fixed.

do you still have the issue or did you get it to come back to you? 

I don't think he ever comes back. Once it's broken, there's no way to complete it.

I hope we will see a bug fix for this soon...

Just had this happen to me. I could have sworn I talked to the C-Sec officer and he thanked me and left, but I suppose I must have reloaded a save or something from before I talked to him because I just checked my quest log two hours of gameplay later and the quest is still not completed, and says I need to talk to the officer.

Thankfully I make a separate save every time I save, so I can just reload, but I've lost two hours of progress as a result of this bug. 

Maybe the C-Sec officer should stick around until players have turned in the quest to him? That'd be a nice fix.
