Well, 'mighty Insanity III' can be achieved by killing Saren three times using a simple save reload, lol. (Edit: dunno if this is still true after June 7.)

But I agree, it's kinda silly to have only one NG+ achievement in the whole trilogy. Also, I'd much rather have two separate trilogy playthroughs with a female and a male Shepard instead of a ME3 NG+ with the same character.
