How to get the Life Preserver or Life Boat Player Card

How to do you get the Life Preserver/ Life Boat Player Card? Please see the attached Player Card picture. I pre-ordered the Ultimate Edition version for the PC. I participated in Alpha/Play testing and Beta. I think it a neat card. Any help will be appreciated.
This was a Twitch Prime reward - I believe you have until the end of the month to claim it but recommend doing that sooner rather than later
