Getting killed AFTER getting into cover is usually a ping thing. Check yours. I have not seen a complaint like yours for a number of years. It used to be very common.
Wall hacks are used still to this day, they can see you through a wall, but they can not shoot you through the wall.
You will have to explain your second question.
The third is the way the game is written. The player tag is called a "BURRITO". A radar hack, or ESP if you prefer, allows the user to see every player, all the time without the need to be "spotted".
Do a speed test and see how you go. speedtest.net is what I use.

Sometimes when I get killed going into cover I think it sometimes its a sync issue with the crossplay. Happens more for me with PS users than xbox or other PCs.

Usually the problem is that due to lag, you are already dead out in the open, but the message only just arrived as you dived into cover. If any player on the server has high ping (including yourself) this tends to make this happen.
The ESP players are usually the ones who drop smoke then jump in and nail everyone, precisely. If you suspect, stay dead and watch how they move\ point the weapon on your death cam.

Thanks interesting did two test one with VPN off and the other on, when off the ping is higher but upload and download and upload take a hit

Dying around cover is part of just lag. It can also be a thing of perspective where if the other guy is further away he can see around the corner more than you would right next to it.
Unfortunately only so much you as a player can do. DNS adjustments and programs like exitlag can be handy but at the end of the day you still need to send info to whatever server the game's on, and the game has to process that for every player every fraction of a second.

Have experienced lag before in games including fps but never as bad as this, guess its just ea lack effort or skillset. So pleased i don't buy their crap. Someone else told me the other player sees different to what i see, in that case how do you aim lol

There are days you move freely; there are days you seem to be moving through syrup- those are the laggy servers. I just came off a server where I emptied a 40 round mag into an AI and killed no one in front of me.
